General consent form for UV Tanning, Red Light, and Sunless Spray Tanning for all members, no matter the service!

Additional consent for our GLO-ers that are under the age of 18 and need permission from a parent or guardian to participate in any and all services. A General Waiver and Guardian Waiver are both required before service.
